The utilization of person-centered care is highlighted as essential for health promotion, yet implementation has been inconsistent and multiple issues remain. There a dearth applied research exploring the facets successful implementation. In this paper, wellbeing program spanning various groups discussed, outlining central principles that have allowed outcomes. Ten years pragmatic pre–post service evaluation shown consistent improvement in measures functional capacity wellbeing. method paper reflective exploration theory practices can explain continual clinics achieved over 10 years.
